By filling out this Donation Request Form, you are requesting a donation from the Altitude Trampoline Park located in BLOOMINGTON, ILLINOIS.  If you are not within 30 miles of Bloomington, IL, please contact your closest Altitude for your request.

We appreciate our customers’ involvement in all the many local non-profit organizations who provide for the needs the our community. Each individual Altitude Trampoline Park has a budget with which to support these projects. Because we get so many requests, and because we have a limited budget, requests are now reviewed by the staff in order to determine our ability to participate. Our availability to participate is based on budget and time of year. Please submit this form 4-6 weeks ahead of time to leave enough time for management to discuss and consider donation request. Due to the large volume of requests we receive, you will only be notified if we can accommodate your request.
